Applications for admission to University Middle shall be subject to verification of the information provided at the time the application is submitted. In order to establish a legal address for University Middle registration, you must provide two approved documents. Independent verification must be provided in the manner specified by University Middle or it will not be accepted. A student’s address shall be the address of the parent(s) or legal guardian(s). If the parent(s) or legal guardian(s) are divorced or legally separated, the address of the parent/legal guardian with physical custody of the child will be used. Custody of a student must be supported by appropriate court documents. By initialing below, I certify that all statements submitted within this application are true and made in good faith. University Middle is authorized to make inquiries and conduct an investigation, if deemed necessary, to verify the information provided. I understand that submission of false or misleading information will cause my child’s enrollment application to be denied. If you are applying under the 2.0 mile radius option, you will be required to supply proof of residency in the form of 2 official documents postmarked or time/date stamped within 30 days of application showing the name and address of the parent of the student on the application.

You will submit this information to the school once you are notified that your application has been accepted for admissions.
