Step 1

  1. Please provide the name and email address of those who will be approvers of the Graduate Applicant Signing Form.
  2. If you are not sure who the approvers should be, please contact your department chair or administrative assistant.
  3. The approvers provided for Department Chair and College Dean/Associate Dean cannot be the same person. Two different email addresses will have to be provided.
  4. Provide contact information for the approvers:
  • Enter the Department Chair's "Name" and "Email" below.
  • Select the "Name" of the College Dean/Associate Dean from the list of Deans. The selection format is "Department Name: Dean's Name."

Step 2

  1. Click the "Continue" button when the form is complete.
